Through this grant, PFCS is able to assist individuals in obtaining prescribed mental health medications while they transition from public services to Medicaid insurance. To qualify for this assistance, an applicant must be in the process of discharge from a hospital, jail, or prison, and be likely to qualify for Medicaid. Our case managers connect eligible individuals with the Department of Social Services and assist with Medicaid paperwork and follow-up.
